If we are not contracted with your insurance, you may contact the office for our out-of-pocket fees.
If your insurance plan is out-of-network and your plan has out-of-network benefits, you may submit a claim to your carrier for reimbursement up to a certain amount, determined by your insurance insurance company. We are happy to provide your a super-bill to assist your in receiving credit towards your deductible. Please send an email to [email protected] with "Out-of-network claim" in the subject line and write the dates of service(s) for which you would like to submit a claim.
Fees are charged for no-shows and late-cancellations (< 24 hours). Please note that these fees are not covered by insurance.
