If we are not contracted (or out-of-network) with your insurance, please view our "Good Faith Estimate" Under the No Surprises Act, effective January 1, 2022. You may also contact the office for any questions related to out-of-pocket fees.

If your insurance plan is out-of-network, we are now able to submit an out-of-network claim to your insurance, as a courtesy, after your visit if we have your up to date health insurance information. The amount your health insurance will reimburse you will be dependent on YOUR plan. 

Fees are charged for no-shows and late-cancellations (< 24 hours).  Please note that these fees are not covered by insurance.

Benefits are based on policy provisions and payment of policy premiums. If claims are denied, the patient will be responsible for all balances accrued.
